Singapore does not place every resident into one health-insurance pool. Start with citizenship or permanent-resident status, then the exact work pass, employer benefit and family policy.
Who receives national cover, and who does not?
MediShield Life (basic national health insurance for Singapore citizens and permanent residents) automatically covers those two groups. CPF (Central Provident Fund, Singapore's mandatory savings system for citizens and permanent residents in covered employment) administers related healthcare savings and premium payments. Ordinary foreign work-pass holders do not become MediShield Life members or CPF contributors merely by working in Singapore.
Public hospitals treat foreign residents, but Ministry of Health subsidies for acute inpatient care are for citizens and permanent residents. A foreign pass holder should budget for unsubsidised charges and ask the hospital and insurer which ward, specialist and pre-authorisation rules apply. Emergency assessment is available, but it should not replace suitable inpatient cover.
Which employer insurance is mandatory?
MOM (Ministry of Manpower, the work-pass authority) requires an employer to buy and maintain medical insurance for every Work Permit and S Pass holder. An S Pass is a work pass for qualifying skilled employees. The policy must cover inpatient care and day surgery, including non-work-related conditions, with an annual claim limit of at least S$60,000 for each worker. The employer cannot pass the purchase cost to the worker.
For claims above the first S$15,000, the required structure has insurers pay at least 75% and employers pay 25%, up to the annual limit. Policies starting, renewing or extending from 1 July 2025 also use standard allowable exclusions, age bands and direct insurer payment to hospitals once a claim is admissible. This minimum protects the employer's medical obligation; it is not proof that every outpatient visit, medicine, dental bill or preferred private hospital is covered.
An Employment Pass is a work pass for qualifying foreign professionals. MOM does not apply that same mandatory S$60,000 insurance rule to Employment Pass holders. A Dependant's Pass is a stay pass for eligible family members of certain work-pass holders, and it does not automatically add the dependant to the employee's plan. Read the employer certificate for every named family member.
What should private or group cover include?
First obtain the employer's benefit schedule, insurer name, hospital network, claim process and end date. Then compare any top-up or individual international medical policy for inpatient and day surgery limits, outpatient clinics, medicines, maternity waiting periods, dental, mental healthcare, pre-existing conditions, deductibles and co-payments. A deductible is the amount you pay before eligible insurer payments; a co-payment is your share after that point.
Ask whether direct billing works at the hospital you would actually use, and whether pre-authorisation is required. Check emergency evacuation and treatment outside Singapore if regional travel matters. Travel insurance is designed around a trip and return home, so it is not a reliable substitute for long-term resident medical cover.
The Monetary Authority of Singapore requires material accident-and-health policy information to be disclosed. The Life Insurance Association warns that exclusions and medical-history disclosure affect claims. Insurers can accept, load, exclude or refuse a pre-existing condition, so there is no honest universal premium or acceptance promise.
What happens when employment or family status changes?
Employer group cover may end on the last working day, pass cancellation or another date in the policy. Ask for that date before resigning and arrange replacement cover without a gap. A move from S Pass to Employment Pass can remove the MOM insurance mandate even if the employer voluntarily keeps benefits.
Permanent residence changes the system: MediShield Life becomes automatic and CPF contributions begin under the permanent-resident rules, but existing private cover still needs review. Marriage, birth, divorce, a dependant's pass change or departure can also alter eligibility, underwriting and cancellation terms.
Common misconceptions
An Employment Pass does not carry compulsory MOM medical insurance under the Work Permit and S Pass rule. Employer cover is also not MediShield Life, and a Dependant's Pass does not make a spouse or child a CPF member or subsidised public-hospital patient.
