South Korea has two main public-insurance routes for newcomers. NHIS, the National Health Insurance Service and public health insurer, records an employee through the workplace or an eligible non-employee as a local subscriber.
Does your employer or residence trigger enrolment?
A foreign employee working at an NHIS-covered workplace is compulsorily enrolled by the employer. Check your first payslip and NHIS eligibility record instead of assuming a contract completed the filing. Employee contributions follow payroll rules and are shared with the employer under the public scheme.
If you are neither employee-insured nor an accepted dependent, mandatory local-subscriber enrolment normally starts after six months of residence for an eligible registered status. NHIS lists E-9 non-professional employment, F-5 permanent residence, and F-6 marriage migration among statuses enrolled from entry, or from foreigner registration if registration occurs later.
Students need their own check. NHIS makes D-2 study and D-4 general-training students subject to mandatory cover, with the applicable start linked to the student and registration rules. An F-4 overseas-Korean student may need to submit enrolment evidence for the student start date. Do not apply a six-month assumption to every student.
Can a family member become a dependent?
A spouse, child, or other permitted relative of an employee-insured person is not added merely because immigration approved a dependant status. NHIS tests the relationship, support, income, property, and, for many foreign dependants entering from April 2024, Korean residence conditions.
The employee files the dependent request with NHIS and provides accepted family records. Foreign documents may need authentication and a Korean translation. An apostille is an international certificate authenticating a public document. Spouses and minor children have an exception from the newer six-month dependent-residence condition, but they still must meet the other dependent rules.
How are premiums, exclusions, and care handled?
NHIS calculates employee premiums from covered pay. Local-subscriber calculations depend on the current statutory method, household position, income or property information, status, and applicable minimum or reduction rules. There is no honest universal foreigner premium, so use the premium notice or ask NHIS for an account calculation.
Private medical insurance is separate from NHIS and does not automatically cancel public enrolment. A foreign resident with qualifying medical protection under foreign law, a pre-existing foreign policy, or an employer contract may apply for statutory exclusion where the rules allow it. NHIS requires translated evidence, limits the exclusion period, and may require renewal. Get written approval before treating private cover as a substitute.
Once active, use the NHIS eligibility record at participating clinics and hospitals. Covered treatment still has patient charges, while non-covered services remain your responsibility. If the start date, dependent decision, premium, or arrears record looks wrong, contact NHIS at 1577-1000 or its English consultation line and use the stated objection or appeal route.
Why does your registered address matter?
NHIS sends eligibility and premium notices to the recorded Korean address. Report a move through immigration within the legal period, then verify the NHIS address separately. Missed mail does not erase premiums.
Leaving Korea, changing employer, changing status, joining as a dependent, or remaining abroad can change the category or end eligibility under NHIS rules. Ask for an updated eligibility certificate after a major change.
Common misconceptions
The Residence Card proves registered identity; it does not itself enrol you in NHIS. Employer reporting, residence duration, status, and dependent tests decide the insurance record.
Private travel cover also does not create a general opt-out. Only an approved statutory exclusion changes compulsory public enrolment.
