The tessera sanitaria is the Italian health card linked to the tax code and health-service record. It is not created merely because you received a codice fiscale. You first need the correct entitlement and registration with the Servizio Sanitario Nazionale, or SSN.
Who can register with the SSN?
The answer depends on nationality and residence basis. Workers, qualifying family members, and several non-EU permit categories have mandatory SSN registration. Some students, elective residents, and other categories may use voluntary registration or private coverage under the rules that apply to them.
EU citizens may register through work, pension, family, or another EU coordination basis. An S1 form can be relevant for certain pensioners or cross-border situations. A temporary EHIC is not the same as becoming a resident member of the Italian system.
Tourists do not normally join the SSN as residents. Non-EU citizens staying temporarily can receive urgent or necessary care under the applicable payment and access rules.
Where do you apply and what do you bring?
Use the ASL, the local health authority for your registered residence or accepted actual domicile. Lombardy uses local structures that may be labelled ATS and ASST, while Lazio, Tuscany, Emilia-Romagna, Sicily, and other regions organise booking and offices differently.
Bring identity, codice fiscale, residence self-certification or domicile declaration, and proof of entitlement. A non-EU applicant can need the valid permesso di soggiorno or the receipt for issue or renewal. Add work, family, S1, student, payment, or other documents required for the category.
What happens after registration?
Choose a medico di medicina generale, or family doctor, from the available local list. Children use a pediatra di libera scelta where applicable.
The health registration is tied to your status and may have an expiry date. Renew it when the permit or entitlement changes. Keep the ASL certificate while waiting for the physical tessera sanitaria.
The card can also carry the European Health Insurance Card function when you qualify. It does not replace travel insurance for every circumstance or guarantee all treatment abroad.
Common misconceptions
One misconception is that a codice fiscale certificate is a health card. The same identifier appears on both, but SSN registration is a separate process.
Another is that healthcare registration works identically in every Italian city. The national framework is delivered by regions and local health authorities.
